Yes — the US taxes its citizens on worldwide income regardless of where they live. US citizens residing in Canada must file a US 1040 every year. The Canada–US Tax Treaty and the Foreign Tax Credit typically eliminate most or all US tax owed, but the filing obligation remains.
FBAR (FinCEN 114) is required for US persons whose foreign bank accounts exceed $10,000 at any point during the year. If you're a US citizen or green card holder with Canadian bank accounts, this almost certainly applies to you. Penalties for non-filing are steep — up to $10,000 per violation.
